Your AI is 99% accurate. Sounds great.

**1,000 decisions/day × 99% accuracy = 10 mistakes per day.**

That's 3,600 mistakes per year. Every one made while the system believed it was right.

And here's the dirty secret: your 99% probably isn't 99% in production. Stanford HAI found hallucination rates between 69-88% on legal queries. OpenAI's o3 exhibited 33-51% hallucination rates on factual recall. The gap between test accuracy and production accuracy can be enormous.

Real-world examples of the 1% going wrong:

- **Air Canada**: Chatbot invented a bereavement discount that didn't exist. Airline held liable by tribunal.
- **Zillow**: Pricing algorithm accumulated $528M in losses in a single quarter before anyone intervened.
- **DPD**: Chatbot wrote poems calling DPD "the worst delivery firm in the world."

Governance doesn't make AI more accurate. It makes AI's *mistakes* less catastrophic — through risk-based routing, confidence thresholds, blast radius limits, and audit trails.
